Responsibilities

Social Worker (LCSW) - Chronic Disease Transition Care

100% fully grant-funded

Dignity Health, Northridge Hospital Medical Center

The Chronic Disease Transitional Care (CDTC) Social Worker LCSW provides care coordination for distress assessment, psychosocial support, community resource allocation, and crisis intervention to patients and families / patient representatives for assigned inpatient units and in the outpatient environment. The LCSW is responsible for coordinating and implementing interdisciplinary care delivery, services and activities related to the Chronic Disease Transitional Care Program. He/she functions as a liaison between the physicians, nurse practitioners, nursing staff, pharmacists, healthcare professionals, patients and their families. Coordinates and facilitates the care for the chronic disease care patient throughout the continuum. The LCSW develops a plan of care based on the application of social work principles & methods to facilitate adjustment to illness, improve coping skills, address end-of-life issues and support during the grieving process.

Full Salary is UniHealth grant funded.

Qualifications

  • Requirements: Masters Degree from accredited school of Social Work, MSW.

  • Licensed Clinical Social Worker (LCSW) in the state of California.

  • Minimum 3 years experience with Chronic Disease, Palliative Care, Oncology, or Hospice Care as an LCSW in an acute care setting required.

  • Advanced Certified Hospice and Palliative Social Worker Certification or equivalent certification required within 6 months from hire/ transfer date into this role.
