Sutter Health Social Worker in Oakland, California
Position Overview:
Our 58-bed Acute Rehabilitation program is CARF-accredited since 1993, having received numerous awards for our comprehensive integrated inpatient rehabilitation program. In addition, we are recognized by CARF for our accredited specialty programs, including stroke, brain injury, spinal cord injury, and cancer rehabilitation.
Under the direction of the program manager and clinical care coordinator(s), the social worker is responsible for assuring that the individual plan of care for the person served is planned and coordinated together with the rehabilitation team and the active involvement of the patient and/or family/caregiver. The social worker functions as a member of the interdisciplinary rehabilitation team and provides psychosocial assessments, resources for the disabled person and discharge planning to patients and their families. Persons served range from early adulthood to late adulthood.
Qualifications:
Education:
- Master’s Degree graduate of an accredited school of Social Work
Licensures and Certifications:
LCSW or MFT licensure in the State of California preferred
Basic Life Support Certification - BLS required
Experience
Previous experience in inpatient/outpatient setting required
Experience with managed care
Skills and Knowledge
Strong interpersonal relationship skills
Demonstrated ability to function independently
Ability to function as a member of multidisciplinary treatment team
Ability to communicate effectively in both oral and written form
Some experience with managed care
Ability to run a variety of groups related to discharge planning, community resources, family work, marital counseling, financial assistance
